What your notice must include
To be effective, your notice must be in writing and include all of the following (17 U.S.C. § 512(c)(3)):

- Identification of the material claimed to be infringing and information sufficient to locate it (such as the full URL).
- Your contact information: full name, mailing address, telephone number, and email address.
- A statement that you have a good-faith belief that the use is not authorized by the copyright owner, its agent, or the law.
- A statement, made under penalty of perjury, that the information in your notice is accurate and that you are the copyright owner or authorized to act on the owner’s behalf.
Counter-notice
If your content was removed and you believe it was a mistake or misidentification, you may submit a counter-notice including:
- Your physical or electronic signature;
- Identification of the material removed and the location at which it appeared before removal;
- A statement, under penalty of perjury, that you have a good-faith belief the material was removed by mistake or misidentification;
- Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the U.S. federal district court for your district (or, if outside the U.S., for any judicial district in which we may be found), and that you will accept service of process from the person who provided the original notice.
Repeat infringers
We will, in appropriate circumstances and in our sole discretion, terminate accounts of users we determine to be repeat infringers.
Misrepresentations
Any person who knowingly materially misrepresents that material is infringing — or that material was removed by mistake — may be liable for damages under 17 U.S.C. § 512(f).

Send your complete notice to dmca@hempplastics.com with the subject line “DMCA Takedown Notice.” We will respond within a reasonable time and may, in our discretion, remove the material in question and notify the user who posted it.
